Tax Forms 税金(ぜいきん) の 証明書(しょうめいしょ) について

If you would like to receive various tax certificates, you can receive them at the Taxation Division at Kyotanabe City Hall or send a request via mail.

税金(ぜいきん) の 証明書(しょうめいしょ)が ほしい 人(ひと)は 京田辺市役所(きょうたなべしやくしょ) の 税務課(ぜいむか) の 窓口(まどぐち) か 郵送(ゆうそう)で 申(もう)し込(こ)む ことが できます。

If you have a My Number Card, you may be able to receive your tax certificates at a convenience store instead.

マイナンバーカード（個人番号(こじんばんごう)カード）を 持(も)っている 人(ひと)は、コンビニで 最新(さいしん)  年度(ねんど) の 税金(ぜいきん) の 証明書(しょうめいしょ)を とる ことが できます。（できない 場合(ばあい)も あります）　

Bring the following to the Kyotanabe City Hall Taxation Division Counter:

1. Form of Personal Identification (My Number Card, Resident Card, Passport, etc.)
2. Processing Fee (300 yen per copy)
3. Completed "Application Form for Tax Certificates" (Available at the Taxation Division Counter)
4. If the applicant wants to receive a tax certificate for someone NOT living in their household, they will need an authorization form. (available below)

下(した) の ものを 京田辺市役所(きょうたなべしやくしょ) の 税務課(ぜいむか) の 窓口(まどぐち)に 持(も)って 来(き)て ください。

1. 申(もう)し込(こ)む 人(ひと) が 本人(ほんにん) か 確認(かくにん)できる もの（在留(ざいりゅう)カード、マイナンバーカード、パスポートなど）
2. 手数料(てすうりょう)（一枚(いちまい) あたり 300円(えん)が かかります。）
3. 書(か)いた 「税関係証明書交付申請書(ぜいかんけいしょうめいしょこうふしんせいしょ)」（京田辺市役所(きょうたなべしやくしょ) の 税務課(ぜいむか) の 窓口(まどぐち)で もらう ことが できます）
4. 同(おな)じ 家(いえ)に 住(す)んでいない 人(ひと) の 証明書(しょうめいしょ)が ほしい 場合(ばあい):　「委任状(いにんじょう)」が 必要(ひつよう)です（下(した) から ダウンロード できます）

Please send the following items in an envelope addressed to the Kyotanabe City Tax Division （〒610―0393　京田辺市田辺80番地　京田辺市役所　税務課)

1. Completed Application Form for Tax Certificates (download available below) 

*If you are unable to download and print the form then please write on paper the following info:

• The current address and past address (their address on January 1st of the fiscal tax year you are requesting) of the person whose certificate you would like.
• The name, furigana, and birthdate of the person whose certificate you would like.
• A day time phone number
• The reason you are requesting this form
• Which fiscal year's certificate and how many copies you would like.

2. A copy of the applicants personal identification (My Number Card, Resident Card, Passport, etc.)

3. A postal money order (定額小為替) of the exact amount in processing fees that you owe. You can purchase a postal money order at the post office. 

4. A return envelope with your return address and the sufficient postage amount in stamps affixed.

＊If you apply via mail, you cannot apply on behalf on someone else and use the authorization form.

下(した) の ものを 『〒610―0393　京田辺市田辺80番地　京田辺市役所　税務課』 に郵便(ゆうびん)で 送(おく)って ください。

1．書(か)いた 申(もう)し込(こ)み書(しょ)　（下(した)で ダウンロード できます）。

※ダウンロードが できない 場合(ばあい)は、 紙(かみ)で 下(した) の ことを 書(か)いて ください。

• 証明書(しょうめいしょ)が 必要(ひつよう) な 人(ひと)が 今(いま) 住(す)んでいる ところ。 前(まえ)に 京田辺市(きょうたなべし)で 住(す)んでいた ところ。（ほしい 証明書(しょうめいしょ) の 年(とし) の 1月(がつ)1日(にち)に 住(す)んでいた ところ）。
• 証明書(しょうめいしょ)が 必要(ひつよう) な 人(ひと) の 名前(なまえ)、フリガナ、誕生日(たんじょうび)。
• 昼間(ひるま)に 連絡(れんらく) できる 電話番号(でんわばんごう)。
• なぜ この 証明書(しょうめいしょ)が ほしい のか。
• いつ の 証明書(しょうめいしょ)が ほしい のか。 何枚(なんまい)  ほしい のか。

2. 申(もう)し込(こ)んでいる 人(ひと)が 本人(ほんにん)と 確認(かくにん)できる もの（マイナンバーカード、在留(ざいりゅう)カード、パスポートなど）の 写(うつ)し

3．手数料(てすうりょう)の分(ぶん)の「定額小為替(ていがくこわせ)」。「定額小為替(ていがくこがわせ)」は 郵便局(ゆうびんきょく)で 買(か)う ことが できます。

4. 今(いま) 住(す)んでいる ところを 書(か)いた 封筒(ふうとう)に 京田辺市役所(きょうたなべしやくしょ)から あなた まで 送(おく)る ため 必要(ひつよう)な 金額(きんがく) の 切手(きって)を 貼(は)って ください。

※郵送(ゆうそう)で 申(もう)し込(こ)む 場合(ばあい)は、委任状(いにんじょう)を 使(つか)う ことが できません。
